TSH measures a signal from your pituitary gland — not what is happening inside your cells. Your pituitary has a uniquely protected conversion system, so TSH can read “normal” while your muscles, brain, and metabolism are starved for active thyroid hormone.
TSH
Useful starting point, but does not reflect tissue-level thyroid function. Can appear normal even when the body is struggling.
Free T4
The inactive form — raw material your body must convert into active T3. High T4 with low T3 signals a conversion problem.
Free T3
The most important marker for how you actually feel. This is what your cells use for energy, metabolism, and brain function.
Reverse T3
An inactive form that blocks T3 at the receptor. Elevated by stress, inflammation, and aging — a hidden driver of symptoms.

What is it?
A slow-release form of active T3 that delivers steady levels throughout the day instead of a single spike.
Why it helps
Commercial T3 (Cytomel) can cause heart racing and energy crashes. Sustained-release avoids these peaks and valleys.
Who may benefit
Patients who need T3 support but are sensitive to standard T3 tablets. Also used in precision microdosing for perimenopause and beyond.
Low Dose Naltrexone (LDN)
What is it?
A very low dose (1.5–4.5 mg) of an immune-modulating medication used off-label to help calm autoimmune thyroid attacks.
Why it helps
Emerging clinical data suggests LDN may help reduce thyroid antibodies and inflammation in Hashimoto’s, addressing the root cause.
Who may benefit
Patients with Hashimoto’s (positive antibodies) who continue to have symptoms despite adequate thyroid hormone replacement.
