Understanding Your Results

Why TSH Alone Isn’t Enough

TSH measures a signal from your pituitary gland — not what is happening inside your cells. Your pituitary has a uniquely protected conversion system, so TSH can read “normal” while your muscles, brain, and metabolism are starved for active thyroid hormone.

Brain Signal

TSH

Useful starting point, but does not reflect tissue-level thyroid function. Can appear normal even when the body is struggling.

Storage Hormone

Free T4

The inactive form — raw material your body must convert into active T3. High T4 with low T3 signals a conversion problem.

Active Hormone

Free T3

The most important marker for how you actually feel. This is what your cells use for energy, metabolism, and brain function.

Metabolic Brake

Reverse T3

An inactive form that blocks T3 at the receptor. Elevated by stress, inflammation, and aging — a hidden driver of symptoms.

Autoimmune Markers

TPO & TgAb Antibodies

Detect Hashimoto’s thyroiditis — the autoimmune cause behind the majority of hypothyroidism cases.

Patterns & What They Mean

Common Patterns That Get Missed

When you look beyond TSH, recognizable patterns emerge that point to specific problems — and specific solutions.

Lab PatternWhat It Likely MeansHow You May Feel
Normal TSH, Low Free T3Conversion problem — your body isn’t turning T4 into active T3Fatigue, weight gain, brain fog, cold hands and feet
Normal TSH, High Reverse T3Stress/inflammation response — T4 being shunted into inactive rT3Exhaustion despite rest, inability to lose weight, sluggish metabolism
Normal TSH, Positive AntibodiesAutoimmune thyroid (Hashimoto’s) — immune system attacking the glandGood days and bad days, joint pain, anxiety, mood swings
Normal Everything, Still SymptomaticTissue-level hypothyroidism — blood levels don’t reflect what cells receive“I feel hypothyroid but my labs are normal” — the hallmark of thyroid aging
Customized for You

Personalized Thyroid Solutions

When standard T4-only therapy isn’t enough, custom compounded formulations can be tailored to your unique physiology — in collaboration with your prescriber.

Custom T4/T3 Combination Capsules

What is it?

A single capsule containing both T4 and T3 in a precise ratio tailored to your needs — more closely mimicking what a healthy thyroid produces.

Why it helps

Research shows some patients feel better with both hormones rather than T4 alone, especially those with conversion issues.

Who may benefit

Patients still symptomatic on levothyroxine, particularly those with low Free T3 despite adequate T4 levels.

Sustained-Release T3

What is it?

A slow-release form of active T3 that delivers steady levels throughout the day instead of a single spike.

Why it helps

Commercial T3 (Cytomel) can cause heart racing and energy crashes. Sustained-release avoids these peaks and valleys.

Who may benefit

Patients who need T3 support but are sensitive to standard T3 tablets. Also used in precision microdosing for perimenopause and beyond.

Low Dose Naltrexone (LDN)

What is it?

A very low dose (1.5–4.5 mg) of an immune-modulating medication used off-label to help calm autoimmune thyroid attacks.

Why it helps

Emerging clinical data suggests LDN may help reduce thyroid antibodies and inflammation in Hashimoto’s, addressing the root cause.

Who may benefit

Patients with Hashimoto’s (positive antibodies) who continue to have symptoms despite adequate thyroid hormone replacement.
